Future Stars Arena

Please read the policies carefully. If you do not agree to the policies please do not register your child.  Thank you for considering a Future Stars Arena program!

RESCHEDULE/CANCELLATION

• Booked sessions can be rescheduled with 72 HOURS NOTICE. If cancellations occur with less than 72 hours notice (for any reason including illness, games, tournaments etc) than the session value will be lost. NO EXCEPTIONS.
   
• There are no refunds or transfers once booked.

PACKAGE EXPIRY DATE

CHRISTMAS CAMPS: January 7, 2012
MARCH BREAK:  March 26, 2012
PASSOVER:  April 14, 2012
PRE-TRYOUT: TBA
SUMMER:  September 1, 12

Purchased sessions/packages must be used in full by the end of the camp program. There will be no credit, transfer, rebooking or refund for any unused sessions. NO EXCEPTIONS.

GENERAL

• Players must be pre-registered and full payment must be provided
• Spots are available on a "first come, first serve basis" and must be pre-booked 48 hrs in advance.
• Future Stars Arena is not responsible for any lost or stolen property including hockey sticks, equipment etc.
• Lunch programs are not nut free. Please advise Maria Pham if your child is allergic to any substance so that proper precautions can be taken by our staff.  Please complete Medical Form.
• Any child considered to be causing a nuisance, bullying, using derogratory language or causing a disruption to other students or staff members will be expelled from the camp without refund or compensation.
• If your child has a Medical Condition please inform Maria Pham and complete a Medical form.
• There are NO REFUNDS, CREDITS or TRANSFERS for any reason including TOURNAMENTS, ILLNESS, HOLIDAYS, BLISTERS etc.
